GUEST AGREEMENT

THIS AGREEMENT INCLUDES A RELEASE OF LIABLITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO SUE THE OWNER OR STAFF OF VETERINARY CLINIC OF MYRTLE BEACH, VETERINARY CLINIC OF MYRTLE BEACH BOARDING AND BATHING, AND ITS ENTITIES. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ AND UNDERSTAND IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT.

In consideration for your pets being permitted to come to VCMB B&B or use any VCMB B&B or VCMB services, you agree to all the policies, procedures, release, terms, and conditions set forth below. This agreement applies to VCMB and its affiliates, employees, owners, agents, and representatives.

All guests must:

- Be current on Rabies, Distemper, Canine Influenza, Bordetella, and fecal (Bordetella, fecal and exam must be completed every 6 months)
- Be in general good health and free of ticks and fleas
- Deemed suitable for boarding by a veterinarian. If your pet is unsuitable, we will give you a full explanation, and offer alternatives
- Must be able to stand and walk on their own
- Be able to tolerate being in an environment with other dogs/cats
- Be leashed when entering and exiting VCMB B&B

Food – Pet(s) will be fed in accordance with the instructions you provide. While at VCMB B&B, guests do not get fed lunch. If you want your pet to eat lunch, you must indicate this at drop off.
Video Release – By using the services offered by VCMB B&B, you agree to allow VCMB and any of their affiliates to use your pet’s name and any image or likeness of your pet taken while at VCMB B&B in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional material.
Personal Property – VCMB B&B will not be liable for any lost, stolen, or damaged personal property belonging either to you or your pet. If your pet causes any damage to the facility, you will be liable for the full cost of all repairs and replacements.
Fees – You must pay for all services at the time you pick up your pet from VCMB B&B. You give your express permission for VCMB B&B to charge any credit card you provided for unpaid services or products upon the date services were supposed to have been completed.
Training – You hereby consent to VCMB’s use of your pet for staff training while your pet is in our care.
Duty to disclose – You must disclose on a continuing basis, any and all medical or other conditions, including but not limited to personality concerns or behavioral incidents that may affect, limit or prevent your pet’s ability to participate in any services. In the event of a material change to the information you provided us, you agree to update your profile and account within 7 days.
Acceptance and acknowledgement of risk – You fully understand that there are inherent and potential risks involved with interactions between humans and dogs which may result in property damage or bodily injury, including, but not limited to, permanent disability, sickness, or death to human or pet AND there may be other risks not known to you or VCMB, nor readily foreseeable at this time (collectively, “Risks”). You fully accept and assume all rRsks and responsibility for all risks, including, without limitation, all losses, costs, and damages incurred as a result of your pet’s participation in our services, including any veterinarian expenses listed in section 9. “Sickness” means any illness, including, but not limited to, Bordetella (kennel cough), bloat, canine flu, or any other form or contagious disease.
Veterinary Care – VCMB B&B will have the right to obtain medical treatment for you pet up to the amount of $____________, if in VCMB B&B’s sole discretion, it appears that your pet may be ill, injured or exhibits any other behavior that would reasonably suggest that your pet may need medical treatment. Reasonable efforts will be made to contact you when VCMB B&B determines that medical treatment is necessary, but if VCMB B&B is unable to contact your or your emergency contact, VCMB B&B and the veterinarian may rely on this Section as your consent to treat your pet. Medical services required will be completed by VCMB.
Immunization records – You agree to provide veterinary records to VCMB B&B upon request, and you hereby grant VCMB B&B to request and obtain veterinary records directly from your Designated Veterinarian to release health and treatment information and immunization records to VCMB B&B staff upon request. You may revoke this authorization at any time upon written notice to your Designated Veterinarian and VCMB B&B.
Signatures – By signing below, you represent and warrant that all the information you provided in this application is complete and accurate to the best of your knowledge. You consent to VCMB B&B’s use of such information for all lawful business purposes which includes but not limited to, providing services to you, operating the VCMB B&B business, marketing to you, communicating with you regarding you pet(s), and using data that includes information about you and your pet for direct marketing. You agree that VCMB B&B may send you email and/or SMS messages regarding your transactions with VCMB B&B, including but not limited to appointment reminders, upcoming reservations, and information and pictures relating to your pet while in VCMB B&B’s care. You further acknowledge and agree that you have read, understand, and agree to all the terms and conditions contained in this agreement, as they may be amended from time to time. You hereby execute this agreement for your pet(s), yourself and your heirs, successors, representatives, and assigns. You further attest that if you are not the owner or sole owner of the pet(s) for who this pet application has been completed, your signature is sufficient to enter into this agreement for and on behalf of any other owner or representative.
